Rising Sun LTC is a Long Term Care Home located in Diamond Valley, Alberta, Canada.
We offer residential care for people who have long-term health concerns.
This service provides care using a collaborative model. The department aims to:

maintain and promote independence and health

prevent illness

ensure dignity and individual self-worth
We maintain an interdisciplinary team approach with residents and their families and provide an environment that encourages physical, psychological and spiritual well-being as well as continuity of care through the coordination of resources and services.
Provides long-term residential care, including:

pharmacy services

Danelle Kilbourn
Long Term Care Manager
As the Care Manager of Rising Sun I oversee the operations of care for residents in a long term care setting. This involves leading a team of health care providers in a variety of disciplines to ensure quality of care and meet legislative requirements. I see this role as an opportunity to serve my local community that I raise my young family in, by providing dignified quality care to our senior population.

Stacy Moore
Nurse Educator
As a Clinical Nurse Educator I support both nursing staff and the multi-disciplinary clinical team. My role includes leadership in the planning, coordination, implementation and evaluation of education for nursing staff including orientation, compulsory and continuing education, and clinical development. I collaborate with management and internal partners to promote standards of clinical, educational and evidence informed practice. I play a key role in providing safe, quality patient and family centered care while reflecting the shared vision and values of AHS.





Julie Snelgrove
Long Term Care Unit Clerk
As a Unit Clerk, Julie is responsible for the efficient flow of communication between health professionals, departments, patients and families while maintaining confidentiality of patient information. She is also responsible for a variety of administrative tasks which may include reception, greeting and directing visitors, patients and health professionals, maintaining patient records and charts, as well as scheduling patient tests and/or treatments. She also performs a range of administrative activities in support of the unit.
Nursing
The nursing department is responsible for the provision of care that promotes the quality of life and considers each resident’s individuality, personal well being, dignity and freedom of choice. The nursing department may be comprised of Registered Nurses (RN), Licensed Practical Nurses (LPN), and Health Care Aides (HCA). Each category of staff is responsible to perform tasks as approved by their professional body or as outlined by the provincial competencies. Please be advised that we are an equal opportunity employer and as such care may be delivered by an opposite gender caregiver i.e. either male or female. Personal care is provided on a 24 hour basis
RN A nurse who has graduated from a formal program of nursing education and has been licensed by an appropriate provincial authority. RN's are here 24 hours a day
LPN A Practical Nurse who has graduated from a formal practical nursing program and has been licensed by an appropriate provincial authority. LPN's are here during Day Shifts 07:00 to 15:15
HEALTH CARE AIDE (HCA) Health Care Aides (previously referred to as nursing aides, nursing attendants or personal care aides) are trained and certified individuals that provide basic health care services and assistance / support with personal care (i.e. dressing, bathing, etc.) for residents. Health care aides are unregulated health care providers who work under the direct or indirect supervision of a regulated nurse or regulated health care professional. 4 HCA's for day shift 07:15 to 15:15, 3 HCA's for evening Shift 15:15 to 23:15, and 1 HCA for Night Shift 23:15 to 07:15
